Our research

We combine long-term patient data, clinical expertise and lived experience to improve life with IBD.

IBD changes over a lifetime. We study that changing course through long-term records from Lothian, the UK-wide PREdiCCt cohort and the experience of people living with IBD. Our clinicians, nurses, researchers and data scientists work together to turn these observations into questions that matter in everyday care.

Four programmes connect the work: Longitude models inflammation over time; Vantage examines what a service can see and act on; Continuum studies health beyond the bowel; and Cairn brings patients into research and conversation. SENTINEL is the platform we are developing to connect this research with more proactive, personalised care.

From understanding to action

Our flagship platform

SENTINEL

Developing clinician-supervised decision support to help teams recognise changing risk and make better decisions with patients.

01 Longitude

Modelling disease in motion

Longitudinal modelling and disease trajectory — inflammation, exposure and risk over time.

Longitude follows inflammation across time. Repeated stool and blood measurements reveal patterns that a single result cannot show. Our published work describes these patterns and their relationship with later outcomes. Current research asks how disease history, cumulative inflammatory burden and dynamic risk models could support earlier recognition of change and more personalised care.

02 Vantage

Seeing risk earlier

Proactive intelligence and observability — the algorithmic layer behind SENTINEL.

Vantage asks how well an IBD service can understand the changing needs of its whole population. It studies which signals are available, where information is missing and how teams could prioritise follow-up. The aim is to identify needs earlier, respect patients’ preferences and make better use of clinical attention through the developing SENTINEL platform.

03 Continuum

Whole-person, whole-course care

Diet, microbiome, ageing, comorbidity, women's health and quality of life across the disease.

Continuum studies the whole person across the course of IBD: diet, the microbiome, other health conditions, ageing and quality of life. Published cohort research provides evidence about flares and health beyond the bowel. Current work asks how these factors interact, which burdens may be modifiable and how care can better reflect what matters to patients.

04 Cairn

Built with patients

Patient partnership, navigation, education and community — the public voice of Edinburgh IBD.

Cairn connects research with the people it is intended to help. Patient events, accessible explanations and opportunities for involvement create space for questions and lived experience. As we develop proactive monitoring and SENTINEL, the programme asks what patients find useful, acceptable and trustworthy, and how their priorities should shape research and future care.

Real-world data

Lothian IBD Registry

Long-term records from approximately 10,000 people cared for in Lothian, supporting research on disease course and outcomes.

The Lothian IBD Registry brings together long-term records from approximately 10,000 people cared for in the region. It supports research on prevalence, inflammatory patterns, treatment and outcomes. Linkage with DataLoch is being developed to extend this work, helping us ask how care and disease course change across whole populations and over many years.

Cohort

PREdiCCt

UK-wide prospective cohort — 2,629 participants, 47 UK recruiting centres — with deep dietary, lifestyle, microbiome and biomarker phenotyping.

PREdiCCt followed 2,629 people with IBD in self-reported remission across 47 UK centres, examining diet, lifestyle and later flares. Its published results link baseline inflammation and some dietary patterns with flare risk. Further work is examining psychosocial factors and the microbiome, with the aim of building evidence that reflects both disease activity and patients’ experience.
